How We Treat This
Bioidentical hormone therapy is the foundation: transdermal estradiol, micronized progesterone, and topical or injectable testosterone when clinically appropriate. Dosing begins conservatively and is adjusted based on symptom response and lab results at 6–8 week intervals. We address sleep, metabolic health, and bone density as part of the same protocol. Menopause affects every system, so the treatment accounts for every system.

Is hormone replacement therapy safe during menopause?
Current evidence supports the safety and benefit of bioidentical hormone therapy when initiated within 10 years of menopause onset or before age 60, particularly transdermal estradiol with micronized progesterone. The risk-benefit profile is individualized. We review your cardiovascular history, breast cancer risk, clotting history, and metabolic status before prescribing, and we reassess regularly.
When should I start HRT for perimenopause symptoms?
You do not need to wait until periods have stopped. Many women benefit from progesterone support and estradiol in the perimenopausal years, when fluctuating hormones cause the most disruptive symptoms. If symptoms are affecting your quality of life and bloodwork supports the diagnosis, treatment can begin.
Can testosterone therapy help women in menopause?
Yes. Low-dose testosterone is prescribed for women experiencing diminished libido, fatigue, and reduced cognitive clarity that persists despite adequate estrogen and progesterone replacement. We dose conservatively, monitor free testosterone and SHBG, and adjust based on symptom response.
How is your approach different from my family doctor's?
We dedicate 60 minutes to your initial consultation. We order a broader laboratory panel than standard practice and track your progress with the Vis Viva Score across subjective symptoms, blood markers, and functional performance.
