How We Treat This
Treatment depends on the underlying cause. Hashimoto's thyroiditis requires immune and inflammatory support alongside thyroid hormone optimization. Patients on levothyroxine (T4) who remain symptomatic may benefit from the addition of liothyronine (T3) or compounded desiccated thyroid. We also address nutrient cofactors essential for thyroid hormone conversion: selenium, zinc, iodine, iron, and vitamin D. Every adjustment follows serial bloodwork and symptom tracking.

FAQ
Why do I still feel tired if my TSH is normal?
TSH alone does not capture the full picture. Your body converts T4 into the active hormone T3, and that conversion can be impaired by nutrient deficiencies, chronic inflammation, or stress. Reverse T3 may be elevated, effectively blocking active thyroid hormone at the cellular level. A full panel including free T3, free T4, and reverse T3 often reveals the cause of persistent symptoms in patients whose TSH appears adequate.
What is the difference between Hashimoto's and hypothyroidism?
Hypothyroidism describes low thyroid function regardless of cause. Hashimoto's thyroiditis is an autoimmune condition in which your immune system attacks the thyroid gland, and it accounts for the majority of hypothyroidism cases in Canada. Identifying Hashimoto's through antibody testing matters because it informs treatment: autoimmune thyroiditis benefits from immune modulation, gut health optimization, and anti-inflammatory strategies alongside standard thyroid hormone replacement.
Do you prescribe T3 or desiccated thyroid in Ontario?
Yes. When clinically indicated, we prescribe liothyronine (T3), compounded T3/T4 combinations, or desiccated thyroid formulations prepared by our Ontario pharmacy partner. Dosing begins conservatively and is titrated based on repeat bloodwork and symptom assessment. T3 therapy requires closer monitoring than T4 alone, and we schedule follow-ups accordingly.
Can thyroid problems cause weight gain and high cholesterol?
Yes. Thyroid hormone regulates basal metabolic rate, lipid metabolism, and thermogenesis. Even mild hypothyroidism can elevate LDL cholesterol and triglycerides, promote weight gain, and impair the body's ability to mobilize stored fat.
