Manus Solis

Polycystic Ovary Syndrome

Restoring metabolic and hormonal order where conventional care often stops at the pill.

Overview

PCOS affects roughly one in ten women of reproductive age and remains one of the most under-treated endocrine conditions in primary care. The diagnosis itself is heterogeneous: some women present with androgen excess and acne, others with insulin resistance and weight gain, and many with overlapping features that resist a single intervention. The standard approach of oral contraceptives and metformin addresses symptoms without examining the underlying metabolic and hormonal architecture.

At Manus Solis, we treat PCOS as the metabolic-endocrine condition it is. We map the full hormonal landscape through comprehensive bloodwork, assess insulin dynamics with fasting insulin and HOMA-IR, and track your progress across all three Vis Viva Score domains. Treatment is individually compounded and titrated to response: how you feel, what your labs show, and what your body can do.

Treatment

How We Treat This

We address each phenotype individually. Insulin-resistant presentations receive targeted metabolic support, including pharmaceutical-grade nutraceuticals, and when indicated, insulin-sensitizing agents. Androgen excess is managed through hormonal optimization rather than suppression alone. Bioidentical progesterone may be prescribed where luteal insufficiency is present. Every protocol is compounded by our Ontario pharmacy partner and adjusted based on serial bloodwork, symptom tracking, and functional metrics.

Women with a confirmed or suspected PCOS diagnosis who want a thorough hormonal and metabolic evaluation beyond oral contraceptives. Patients who have been told to 'just lose weight' and want a physician who will investigate the reasons weight loss is difficult in the first place.

Questions

FAQ

Can PCOS be treated without birth control pills?

Yes. While oral contraceptives can regulate cycles and reduce androgens, they do not address the metabolic root of PCOS. Our approach targets insulin resistance, hormonal imbalance, and inflammation individually. Many patients achieve regular cycles and symptom relief through metabolic optimization, bioidentical progesterone, and targeted supplementation without relying on oral contraceptives.

How does insulin resistance relate to PCOS?

Insulin resistance drives much of the hormonal disruption in PCOS. Elevated insulin stimulates the ovaries to produce excess androgens, which in turn causes acne, hirsutism, and irregular ovulation. Addressing insulin resistance through diet, exercise, and where appropriate, pharmaceutical agents, often improves hormonal balance without direct hormonal intervention.

What bloodwork do you order for PCOS in Ontario?

We order a comprehensive panel that goes well beyond the standard TSH and fasting glucose. This includes fasting insulin with HOMA-IR calculation, full androgen profile (total and free testosterone, DHEA-S, SHBG), LH/FSH ratio, estradiol, progesterone, prolactin, AMH, thyroid panel, inflammatory markers, lipids, HbA1c, and key micronutrients. All requisitions are available to Ontario residents.

Can I do PCOS consultations virtually in Ontario?

Yes. All consultations are available virtually to patients anywhere in Ontario. Bloodwork requisitions are sent to your local LifeLabs or Dynacare, and compounded prescriptions ship directly from our Ontario pharmacy partner. Follow-up visits and ongoing titration are managed through secure virtual appointments.
